Requesting Your Records
You have the right to request access to your health records. To do so, please submit your request in writing by contacting me directly at dwhite@amidstitalltherapy.com. I will respond in accordance with Texas law and HIPAA guidelines.

Licensing & Regulatory Questions
If you have questions about my license or the regulation of counseling services, you may contact the Texas Behavioral Health Executive Council.

Filing a Complaint
If you wish to file a formal complaint, you may do so through the Texas Office of the Attorney General.
